e6036b8266 Clean up smarty templates. 
This simplifies some logic in if-conditions, because
smarty just returns an empty string for undefined
variables.

Also, this commit removes unnecessary values from
HTML input attributes.

5d1d0d1a9f Remove empty help spans from templates. 
The empty spans from the templates mess up the layout sometimes, and are useless weight anyway.
Lets get rid of them.
